A physician will be onsite at Dover High School on Tuesday, 9/6 in order to provide physicals for student athletes that have no been able to attain a physical for the upcoming athletic seasons.
Please complete the following Student Physical Permission Form by September 2, 2022, and return it to the High School Office or Athletic Department. You can also email the form to Andrea Mina, School Nurse at Andrea.Mina@doverschools.org
